Physicians, nurse practitioners and sleep clinics: fax the prescription to the CPAP Central store nearest your patient, and we take it from there.

How it works
What happens after you fax
You send one fax. We handle the appointment, the fitting and the funding paperwork, and we tell you when your patient is on therapy.
Step one
You send the referral
Fax the prescription to the store nearest your patient. The four numbers are below.
Step two
We book the fitting
We contact your patient, arrange the appointment and fit the therapy in person.
Step three
We finish the paperwork
CPAP Central is a registered Ontario ADP vendor, so we complete the ADP submission and confirm with you once therapy has started.

Choose the store nearest your patient. All four offer home visits, so distance does not have to decide the referral.

What to send
- Prescription with the pressure settings
- Sleep study report
- Patient name, date of birth and contact details
- ADP authorizer details, if you have them

If travel is difficult for your patient, all four stores fit therapy at home. Note it on the referral and we will arrange it when we call to book.
